Your oral health is extremely important and should not be neglected. Moorman, Lindsey, & Crawford Dental accepts adults and children as new patients and is available to treat your entire family. Scheduled appointments as well as emergency care are available. Please call for more information, so we will be prepared for your visit.
To expedite your initial visit, new patient forms are provided for you below. Simply click the link below to complete the new patient paperwork prior to your first appointment. Remember to bring your dental insurance card if applicable. If for any reason you cannot keep a scheduled appointment, please contact us at least 24 business hours in advance.
We are happy to provide phone, text, or email reminders of your upcoming scheduled visit. We also send reminder postcards to aid in scheduling your family’s cleanings or for your scheduled dental care appointments.
